for our new patients

Please arrive about 15 minutes prior to your appointment so that you can complete the necessary paperwork. If you'd prefer to complete some of the forms prior to your arrival, they are included in the "printable forms" section of this website.

What to bring to your appointment

  • Your insurance and/or Medicare cards
  • Your medications in original bottles - Including over-the-counter vitamins/herbal supplements
  • Pertinent information about your medical and surgical history

When You Arrive

When you arrive for your appointment, please check in with the receptionist.
If you are ever left to wait more than 20 minutes, please check back with the receptionist.

We try to adhere to scheduled visit times, but emergencies may delay the doctor's schedule. We apologize if this should happen and want you to know we respect your time and value you as our patient.

Return Appointment

At the end of each visit, ask your physician or medical assistant (MA) when you should plan to return. This enables you to schedule your next appointment before you leave the office.
